Life Hospice, Inc. provides services to terminally ill patients with a wide range of diagnoses.

"Patient services are provided without regard to race, color, religion, age, gender, sexual orientation, disability (mental or physical), communicable disease, or place of national origin.

In keeping with the hospice concept, Life Hospice, Inc. provides intensive treatment of symptoms. The palliative care focuses on enhancing patient's comfort and overall quality of life and even if care is provided in the patient’s home.

Services are available 24 hours a day, 7 days a week on an intermittent basis

Care Giving Team


Attending Physician

certifies the patient has a terminal illness and remains the patient's primary physician and works as a member of the hospice team.

Medical Director

Certifies the attending physician's prognosis. Leads the hospice team in developing a plan of care: provides consultation to the attending physician.

Skilled Nurses RN's & LVN's

Evaluates the patient’s condition during regularly scheduled visits. The registered nurse coordinates the plan of care and teaches the family proper patient care techniques the use of medication and pain management.

Social Worker

Assess social needs of the patient and family. and provides psychological and bereavement counseling.

Chaplain

Provides spiritual support. Works as a liaison with the clergy of the patient's choice.


Admission Criteria

The patient must be eligible for hospice and agree that they wish to receive palliative rather than curative care.
